Zwar war die unter anderem in Weidenrinde enthaltene Salicylsäure schon seit einigen tausend Jahren als schmerzstillendes, fiebersenkendes und entzündungshemmendes Naturheilmittel bekannt, hat aber gravierende Nachteile: Sie schmeckt äußerst unangenehm, verursacht Brechreiz und greift bei längerer Einnahme die Magenschleimhaut an. Wie erste Tests in der Pharmakologischen Abteilung zeigen, minimiert die Zumischung von Essigsäure nun tatsächlich die unerwünschten Nebenwirkungen, sodass der Leiter der Forschungsabteilung, Arthur Eichengrün, auch gegen anfängliche Widerstände im eigenen Haus, eine klinische Studie durchsetzt, die die positiven Eigenschaften bestätigt. Im März 1899 wird die neue Substanz schließlich unter dem Handelsnamen „Aspirin™“ ins Handelsregister eingetragen, ein Jahr später auch in den USA ein Patent erteilt. Das Medikament wird zunächst als Pulver in 250-Gramm-Glasflaschen angeboten, um sich von Nachahmern abzuheben, bringt Bayer es innerhalb nur eines Jahres in neuartiger Tablettenform auf den Markt, die eine einheitliche Dosierung erlaubt.
In den 1940er-Jahren meldet Eichengrün Ansprüche auf die Urheberschaft der Acetylsalicylsäure an, weil er als Vorgesetzter die eigentliche Konzeption der Entwicklung vorgenommen sowie die Testung des Präparats vorangetrieben und Hoffmann seine Vorgaben lediglich umgesetzt habe. Die Frage, wer als eigentlicher Erfinder zu betrachten ist, ist daher umstritten.
